ABSTRACT

Objective: to review the literature on the relationship between oral health and bariatric surgery in obese individuals. Methods: articles published until March 2021 were searched in the Medline/PubMed, LILACS, BBO, and Web of Science databases. The search used a combination of descriptors related to the words: obesity, bariatric surgery, and oral health. Results: one hundred thirty-five studies were identified in the database search. Of these, 47 were included in this review. The oral changes investigated in the literature for individuals who underwent bariatric surgery were dental caries, tooth wear/erosion, hypersensitivity, periodontal disease, hyposalivation, halitosis, and changes in mastication and oral soft tissue. The evaluated articles showed methodological controversy regarding the study design, follow-up period, sample size, and assessed clinical parameters, which complicated the comparison of studies. Final considerations: there is no consensus in the literature regarding the relationship between oral health and bariatric surgery in obese individuals. Further prospective cohort studies should be conducted to investigate this association better.(AU)


Objetivo: revisar a literatura sobre a relação entre saúde bucal e cirurgia bariátrica em indivíduos obesos. Métodos: foi realizada uma busca de estudos publicados até março de 2021 nas bases de dados Medline/PubMed, LILACS, BBO e Web of Science. Foi utilizada uma combinação de descritores relacionada às palavras: obesidade, cirurgia bariátrica e saúde bucal. Resultados: foram identificados 135 estudos nas bases de dados pesquisadas. Desses, 47 estudos foram incluídos nesta revisão. As alterações bucais investigadas em indivíduos que realizaram cirurgia bariátrica apresentadas na literatura foram: cárie dentária, desgaste dentário/erosão dentária, hipersensibilidade, doença periodontal, hipossalivação, halitose, alterações da função mastigatória e alterações no tecido mole bucal. Os estudos avaliados mostraram controvérsia metodológica em relação a: tipo de desenho de estudo, tempo de seguimento, tamanho da amostra e parâmetros clínicos avaliados, fato que dificultou comparar os estudos. Considerações finais: não há consenso na literatura sobre a relação entre saúde bucal e cirurgia bariátrica em indivíduos obesos. Mais estudos de coorte prospectivos devem ser realizados para estudar melhor essa associação.(AU)

ABSTRACT

Objetivo: avaliar quais as possíveis causas e quais são os fatores associados ao surgimento da halitose. Métodos: foi realizada uma revisão nos bancos de dados PubMed e SciELO, utilizando os descritores e sinônimos MeSH organizados em lógica booleana: 'Halitosis' AND 'Risk Factors' OR 'Association' OR 'Etiologies', com as seguintes etapas: identificação dos artigos, triagem, elegibilidade e inclusão. Apenas artigos publicados entre os anos de 2015-2020, ensaios clínicos randomizados, estudos transversais ou de coorte foram incluídos na revisão. Foram excluídos protocolos de estudos e os artigos que não estavam nos critérios e objetivo desta revisão. Resultados: de 240 artigos encontrados, 22 foram inclusos. Desses, 50% demonstraram que a falta ou a deficiência dos hábitos de higiene oral se relacionam com a halitose. Ainda, 40,9% deles se referem à saburra lingual como um forte fator etiológico e 36,4% demonstram que a doença periodontal está diretamente associada com o mau hálito. Além disso, em relação a outras condições locais e sistêmicas, xerostomia (13,6%), doença gastrointestinal e refluxo laringofaríngeo (9,1%), rinite e diabetes (4,5%) também demonstraram associação. Considerações finais: a halitose pode estar associada com higiene bucal, saburra lingual, doença periodontal, alguns hábitos e quadros sistêmicos.(AU)


Objective: to evaluate the possible causes and what are the factors associated with the onset of halitosis. Methods: a review was performed in the PubMed and SciELO databases, using the descriptors and synonyms MeSH organized in Boolean logic: 'Halitosis' AND 'Risk Factors' OR 'Association' OR 'Etiologies', with the following steps: identification of articles, screening, eligibility and inclusion. Only articles published between 2015- 2020; randomized clinical trials; cross-sectional or cohort studies were included in the review. Study protocols and articles that were not in the criteria and objective of this review were excluded. Results: Of 240 articles found, 22 were included. Of those, 50% showed that the lack or deficiency of oral hygiene habits are related to halitosis. Furthermore, 40.9% of them refer to tongue coating as a strong etiological factor and 36.4% demonstrate that periodontal disease is directly associated with bad breath. Moreover, in relation to other local and systemic conditions, xerostomia (13.6%), gastrointestinal disease and laryngeal reflux (9.1%), rhinitis and diabetes (4.5%) also demonstrated an association. Final considerations: Halitosis may be associated with oral hygiene, lingualcoating, periodontal disease, some habits and systemic conditions.(AU)

RESUMO Introdução: A halitose é um sinal da presença de mau cheiro ofensivo ao olfato humano, geralmente é provocada pela falta de higiene ou estilo de vida e afeta milhares de pessoas em todo o mundo. Objetivo: Realizar uma revisão de literatura a respeito da etiologia, diagnóstico e tratamento da halitose buscando proporcionar um maior embasamento cientifico com informações atualizadas para os profissionais da área. Métodos: O estudo caracterizou-se por uma busca de artigos na base de dados eletrônicos PubMed, Lilacs, Google Acadêmico e Scielo publicados entre 2014 e 2019, em inglês, português e espanhol. Foram consultados 52 trabalhos, destes, 28 foram selecionados após uma criteriosa filtragem. Análise e integração das informações: Diversas etiologias advindas da cavidade bucal podem provocar a halitose. Existem vários exames complementares para auxílio diagnóstico da halitose, tais como testes sialométricos, organoléptico e a análise de presença de saburra para o diagnóstico. Há diversos protocolos instituídos para o tratamento da halitose, desde procedimentos preventivos a curativos. Conclusão: No que concerne a etiologia, a halitose pode ocorrer por fatores fisiológicos ou patológicos; 90 porcento dos casos tem como origem a cavidade oral, podendo estar associada ao biofilme dentário, cárie, língua saburrosa, próteses mal adaptadas e cicatrização tecidual. O diagnóstico deve ser realizado por meio de um exame clínico criterioso. A utilização de questionários para colher informações como frequência, duração, quantidade de vezes que aparece no mesmo dia, hábitos e medicamentos utilizados ajudarão na elucidação do caso. O sucesso do tratamento depende do diagnóstico e da implementação de uma terapia relacionada com a causa-efeito e de uma abordagem multidisciplinar racional que se torna essencial para a obtenção de bons resultados, com objetivo de melhorar a saúde oral, bem como, seus efeitos individuais e sociais(AU)

Objetivo: Llevar a cabo una revisión de la literatura sobre la etiología, el diagnóstico y el tratamiento de la halitosis buscando proporcionar una mayor base científica con información actualizada para los profesionales en el campo. Métodos: El estudio se caracterizó por una búsqueda de artículos en la base de datos electrónica PubMed, Lilacs, Google Scholar y SciELO publicados entre 2014 y 2019, en inglés, portugués y español. Se consultaron 52 documentos, de los cuales 28 fueron seleccionados después de un filtrado cuidadoso. Análisis e integración de las informaciones: Diversas etiologías provenientes de la cavidad bucal pueden provocar la halitosis. Existen varios exámenes complementarios para ayudar a diagnosticar la halitosis, tales como pruebas sialométricas, organolépticas y el análisis de presencia de saburra para el diagnóstico. Hay varios protocolos instituidos para el tratamiento de la halitosis, desde procedimientos preventivos a curativos. Conclusiones: En cuanto a la etiología de la halitosis puede ocurrir debido a factores fisiológicos o patológicos. Aunque el 90 por ciento de los casos se originan en la cavidad bucal y pueden estar asociados con biopelículas dentales, caries, toques de lengua, prótesis mal adaptadas y cicatrización de tejidos. El diagnóstico debe hacerse mediante un examen clínico cuidadoso. El uso de cuestionarios para recopilar información como la frecuencia, la duración, la cantidad de veces que aparece el mismo día, los hábitos y los medicamentos utilizados ayudarán a dilucidar el caso. El tratamiento exitoso depende del diagnóstico y la implementación de la terapia de causa y efecto y de un enfoque multidisciplinario racional que es esencial para obtener resultados exitosos para mejorar la salud bucal y sus efectos individuales y social(AU)


ABSTRACT Introduction: Oral halitosis or malodor is offensive to the human sense of smell. It is often the result of poor hygiene or lifestyle and affects thousands of people worldwide. Objective: Conduct a literature review about the etiology, diagnosis and treatment of halitosis with the purpose of contributing a broader scientific basis of updated information for professionals in the field. Methods: The study was characterized by a search for papers published in the electronic databases PubMed, Lilacs, Google Scholar and Scielo between 2014 and 2019 in English, Portuguese or Spanish. A total 52 papers were consulted, of which 28 were selected after careful filtering. Data analysis and integration: Halitosis may be due to a variety of oral cavity etiologies. Several complementary tests are available which help diagnose halitosis, e.g. sialometric tests, organoleptic tests and analysis of the presence of saburra for the diagnosis. Several protocols have been implemented for the treatment of halitosis. These range from preventive to healing procedures. Conclusions: The etiology of halitosis may be due to physiological or pathological factors. Although 90 percent of the cases originate in the oral cavity and may be associated with dental biofilm, caries, tongue saburra, ill-fitted prostheses or tissue scarring, diagnosis should be based on careful clinical examination. Using questionnaires to gather information such as frequency, duration, number of times it appears on the same day, habits and medications will help elucidate the case. Successful treatment depends on the diagnosis and implementation of a cause-and-effect therapy and a rational multidisciplinary approach which is essential to obtain positive outcomes and improve oral health as well as its individual and social effects(AU)

RESUMEN Halitosis es la presencia de un olor desagradable que emana de la cavidad oral. La etiología de este mal olor es multifactorial, existiendo causas no orales, orales y extraorales. Los compuestos volátiles sulfurados (CVS): sulfuro de hidrógeno y metilmercaptano son los elementos prominentes del mal olor. Intraoralmente, existe una correlación significativa entre las concentraciones de CVS y bacterias periodontopatogénicas. En pacientes sin enfermedad periodontal el mal olor se origina principalmente de la cobertura del dorso de la lengua por lo que la limpieza o raspado de ésta es un método efectivo para prevenir o disminuir la halitosis. Existen múltiples opciones químicas para el abordaje del mal aliento que dependerán del origen y grado de severidad. Este artículo revisa el origen y los tratamientos tanto mecánicos como químicos que se tienen disponibles.


ABSTRACT Halitosis is the presence of an unpleasant smell that emanates from the oral cavity. The etiology of halitosis is multifactorial, existing non oral, oral and extraoral causes. The volatile sulphur compounds (VSC): hydrogen sulfide and methyl mercaptan are the prominent elements of malodour. Intraorally there is a significant correlation between VSC concentrations and periodontopathogenic bacteria. In patients with no gum disease the halitosis is originated mainly in the dorsal surface of the tongue, so cleaning or brushing it is an effective method to prevent or decrease halitosis. There are multiple chemical options for the treatment of the malodour that will depend on the origin and severity of it. This article reviews the origin and mechanical and chemical treatments that are available.

ABSTRACT

RESUMEN La halitosis es un motivo de consulta frecuente y que puede repercutir de gran manera en la calidad de vida de quien la presenta. Puede dividirse en halitosis genuina, pseudohalitosis y halitofobia. De las halitosis genuinas, cerca del 90% son secundarias a patología de la cavidad oral. En otorrinolaringología, las principales causas responden a tonsilolitiasis, patología rinosinusal y cuerpos extraños nasales. Existen distintos métodos diagnósticos que permiten medir e identificar la halitosis, con sus beneficios y desventajas. Las medidas generales de higiene oral, así como intervenciones específicas deben estar basadas en la evidencia a cargo de un equipo multidisciplinario que permita garantizar mejores resultados.


ABSTRACT Halitosis is a frequent health problem, that can have a huge impact in the quality of life of those affected. It can be classified as genuine halitosis, pseudohalitosis and halitophobia. Of genuine halitosis, approximately 90% is originated from the oral cavity. In otorhinolaryngology, most frequent causes are tonsiloliths, rhinosinusitis and nasal foreign bodies. There are different diagnostic methods, each one with its benefits and disadvantages. General measures of oral hygiene, as specific interventions must be evidence-based in charge of a multidisciplinary team that guarantees better results.

ABSTRACT

Objective: To perform the transcultural adaptation to Brazil and validation of the Halitosis Associated Life-quality Test (HALT) for use in adolescents. Material and Methods: Students aged 12 to 15 years, of both sexes, with healthy permanent dentition, attending public schools in Piracicaba, Brazil, were selected for this study. The HALT was translated, backtranslated, reviewed by an expert committee and submitted to a pre-test. In the first phase (n = 13), 15% of the sample did not understand items 2, 12 and 15; therefore, these items were reformulated and resubmitted to 13 adolescents. The version with a sociocultural adaptation was self-applied by 56 adolescents (64% females, 13.4 ± 1.0 years), of whom 25 participated in the test-retest. The presence of visible biofilm, gingival bleeding, calculus and tongue coating, was evaluated. Selfperception of halitosis after exhalation/breathing was assessed by a single question. Results: The HALT score ranged from 0 to 62 points (mean = 10.1±13.4) and the floor effect was 5.4%. Internal consistency was excellent (Cronbach's alpha = 0.92) and reproducibility was moderate (Intraclass Correlation Coefficient = 0.59). Individuals with "excessive" tongue coating had a higher HALT score than those with mild and/or moderate coating (p<0.001). Multiple linear regression analysis indicated that only the male sex was related to a higher HALT score (p<0.05). Conclusion: The Brazilian Portuguese version of the HALT was easily understood by adolescents. Those with greater tongue coating accumulation reported more frequently the presence of halitosis, which was only associated to male sex.

Abstract Halitosis is still poorly studied in young adults. The aim of this study was to evaluate the occurrence of self-reported halitosis and associate it with demographic and behavioral factors in young adult dental students. This cross-sectional study was designed as a census of students enrolled in three initial and three final semesters of a dental course in a Brazilian public university. Of 284 eligible students, 257 (90.5%) completed a self-administered questionnaire. Self-reported halitosis was the primary study outcome, and was assessed with the question "do you feel you have bad breath?". Data on age, gender, frequency of tooth brushing and interproximal cleaning, tongue cleaning, mouth rinse use and dry mouth were collected using the questionnaire, and were considered independent variables. Of the students surveyed, 26.5% reported as never, 51.7% as rarely, 21.4% as sometimes, and 0.4% as always feeling they had halitosis. Morning halitosis was reported by 90.6% of those who reported halitosis. In the final multiple model, last semester students had a 55% lower chance of reporting halitosis, compared with students from the first semesters [odds ratio (OR) 0.46; 95%CI 0.24-0.89]. Women had a 2.57fold higher chance of reporting halitosis (OR = 2.57; 95%CI 1.12-5.93). Dry mouth increased the chance of self-reported halitosis 3.95-fold, compared with absence of dry mouth (OR = 3.95; 95%CI 2.03-7.68). It can be concluded that self-reports of halitosis were low among dental students, but may represent an important complaint. Gender, dry mouth and level of college education of the dentist were factors significantly associated with self-reported halitosis.

ABSTRACT

Halitosis generally refers to the unpleasant odor of breath irrespective of its origin. Oral malodour could result in diverse problems in daily life such as social embarrassment and can adversely affect individuals' social interactions. Though many oral and non-oral sources could give rise to halitosis, it was mainly associated with oral cavity conditions and volatile sulfur compounds -produced by microbial activity- were the main elements of oral malodor. Objective measurement was the first step in assessment to determine presence of malodour. Then, taken a complete history including diet and habit history and performing a comprehensive physical examination contribute to the primary two steps for evaluating a patient complaining. The oral malodor management was mainly achieved by determining and eliminating the etiology of the condition. A major step in this regard was improving the oral health by means of establishing appropriate oral hygiene measures and controlling tongue flora by brushing or scraping and also, use of antiseptics as adjuvant therapy. Current article was systematic reviews the literature on prevalence, classification, diagnosis and treatment of halitosis

ABSTRACT

La halitosis corresponde a una enfermedad muy prevalente en nuestro medio. Presentando gran importancia en nuestra sociedad actual, debido a los altos estándares de higiene e importancia de la imagen propia, puede traer consecuencias psicológicas importantes a quienes la padecen. Más del 85% de la halitosis se debe a la acción de bacterias anaerobias, quienes a través de la degradación de proteínas producen compuestos llamados CSV, que producen el mal olor. El 98% de las causas de halitosis se debe a enfermedades donde es necesaria la evaluación de profesionales de la salud bucal y otorrinolaringólogos, quienes deben trabajar en conjunto para una recuperación óptima del problema. Las mediciones organolépticas siguen siendo el gold standard. Aquí se presenta un novedoso método de screening medidor, el medidor B/B, el que ha mostrado buenos resultados. Se muestra en esta revisión un algoritmo diagnóstico propuesto en la Revista de Otorrinolaringología de Brasil, quienes recomiendan este protocolo diagnóstico para minimizar costos y optimizar recursos. Permitiendo un tratamiento adecuado y buen pronóstico.


Halitosis is a very prevalent disease in our environment, with great importance in our society, due to the high standards of hygiene and the importance of self-image, it can bring important suffer and psychological consequences. Over 85 % of halitosis is produced by the action of anaerobic bacteria, who through protein degradation produce the liberation of compounds called CSV that produce odor. A 98% of the causes of halitosis are due to diseases where the assessment of oral health professionals and otolaryngologists is important, and who must work together for optimal recovery of the problem. The organoleptic measurements remain the "gold standard." In this review a novel method of screening meter is presented, the meter B/ B, which has shown good results. This papers shown a proposed a ENT diagnostic algorithm, who recommend a diagnostic protocol to minimize costs and optimize resources, allowing proper treatment and good prognosis.

OBJECTIVE: To investigate the determinant factors of discomfort attributed to the use of fixed orthodontic appliance and the effect on the quality of life of adolescents. MATERIAL AND METHODS : Two hundred and seventy-two individuals aged between 9 and 18 years old, enrolled in public and private schools and undergoing orthodontic treatment with fixed appliance participated in this cross-sectional study. The participants were randomly selected from a sample comprising 62,496 individuals of the same age group. Data was collected by means of questionnaires and an interview. Discomfort intensity and bio-psychosocial variables were assessed using the Oral Impact on Daily Performance questionnaire. Self-esteem was determined using the Global Negative Self-Evaluation questionnaire. Statistical analysis involved the chi-square test and both simple and multiple Poisson regression analyses. RESULTS: Although most individuals did not present discomfort, there was a prevalence of 15.9% of impact on individuals' daily life exclusively due to the use of fixed orthodontic appliance . Age [PR: 3.2 (95% CI: 1.2-8.5)], speech impairment [PR: 2.2 (95% CI: 1.1-4.6)], poor oral hygiene [PR: 2.4 (95% CI: 1.2-4.8)] and tooth mobility [PR: 3.9 (95% CI: 1.8-8.1)] remained independently associated with a greater prevalence of discomfort (P ≤ 0.05). CONCLUSIONS: Discomfort associated with the use of fixed orthodontic appliances exerted a negative influence on the quality of life of the adolescents comprising the present study. The determinants of this association were age, poor oral hygiene, speech impairment and tooth mobility. .


OBJETIVO: investigar os fatores determinantes do desconforto atribuído ao uso do aparelho ortodôntico fixo e sua influência na qualidade de vida de adolescentes. MÉTODOS: participaram desse estudo transversal 272 indivíduos, com idades entre 9 e 18 anos, estudantes de escolas públicas e privadas, que usavam aparelhos ortodônticos fixos (braquetes). Os participantes foram selecionados aleatoriamente, entre 62.496 indivíduos da mesma faixa etária. A coleta de dados foi feita em forma de entrevista e questionário. A intensidade do desconforto e variáveis biopsicossociais foram avaliadas por meio do Oral Impact on Daily Performance (OIDP). A autoestima foi determinada pela Global Negative Self-Evaluation (GSE). A análise estatística envolveu o teste qui-quadrado e a análise de regressão de Poisson, simples e múltipla. RESULTADOS: embora a maioria dos indivíduos não apresentasse desconforto, observou-se uma prevalência de impacto na vida diária devido, exclusivamente, ao uso do aparelho ortodôntico fixo, de 15,9%. Variáveis idade de 15 a 18 anos [RP = 3,2 (IC 95% = 1,2-8,5)], dificuldade de falar [RP = 2,2 (IC 95% = 1,1-4,6)], dificuldade de limpar a boca [RP = 2,4 (IC95% = 1,2-4,8)] e mobilidade dos dentes [RP = 3,9 (IC 95% = 1,8-8,1)] permaneceram associadas, de forma independente, à maior prevalência de desconforto (p ≤ 05). CONCLUSÕES: desconforto associado ao uso de aparelhos ortodônticos fixos influenciou negativamente a qualidade de vida de adolescentes. Os fatores determinantes foram idade, dificuldade de limpar a boca, de falar e mobilidade dentária. .

ABSTRACT

La halitosis es una afección muy prevalence en la población mundial que genera un gran impacto en la calidad de vida de los pacientes y es un motivo frecuente de consulta en la práctica clínica diaria en otorrinolaringología, presentándonos un gran desafío para el manejo de esta condición. En general se considera que es exclusiva de la odontología, por lo que resulta interesante revisar en este artículo los factores etiológicos involucrados y los avances en diagnóstico y tratamiento.


Halitosis is a very prevalent disease in the world population that generates a great impact on the quality of life of patients and is a frequent complaint in daily clinical practice in otolaryngology, presenting us with a great challenge for the management of this condition. It is generally considered to be exclusive of dentistry, so it is interesting to review in this article the etiological factors involved and advances in diagnosis and treatment.

La halitosis, también conocida como mal olor de la boca, es una condición común que afecta millones de personas. Este término describe una gama de olores desagradables emitidos en el aliento, con una variedad de causas que usualmente resultan de la putrefacción bacteriana dentro de la cavidad bucal. Miyazaki y cols.1 publicaron una clasificación de los diferentes tipos de halitosis en tres categorías: pseudohalitosis, halitofobia y halitosis genuina. Frecuentemente las personas que sufren de mal aliento lo desconocen, mientras que otros están convencidos que sufren de mal olor de la boca aunque no exista tal evidencia, es decir, muchas personas emiten mal aliento por años sin percatarse y otras sobreestiman su propio mal olor, teniendo actitudes obsesivas y evitando las interacciones sociales. Existen varios métodos para evaluar la halitosis y muchos investigadores usan métodos combinados para identificar sujetos con halitosis. Este artículo está basado en la revisión bibliográfica de las técnicas y estrategias utilizadas en el análisis clínico del mal olor de la boca, en razón que los pacientes con halitosis, frecuentemente no tienen una base objetiva y esperan un diagnóstico por parte del odontólogo.


Bad breath, also known as halitosis, is a common condition concern for millions of people. It is a broad term describing a range of unpleasant or offensive odors emitted in the breath, which can have a variety of causes, usually by result of microbial putrefaction within the oral cavity. Miyazaki et al.1 were published a classification system for different types of halitosis in three categories: pseudohalitosis, halitophobia and genuine halitosis. Often, people suffering from bad breath remain unaware of it, whereas others remain convinced that they suffer from foul oral malodor, although there is no evidence for such. Many people emit bad breath for years without being at all aware of it and others greatly overestimate their own oral malodor and are consequently prone to obsessive behavior, avoid social interactions. Several methods are available in the market for the measurement of halitosis. Most clinicians and researchers have used a combination methods assessment to identify subjects with halitosis. In this article, we focus on the clinical techniques and strategies used for the clinical analysis of oral malodor, because, the patient's complaint of bad breath does not appear to have an objective basis and expect effective diagnosis from dental practitioner.

ABSTRACT

To determine whether site-specific mouth rinsing with oral disinfectants can improve oral odor beyond the traditional panoral mouth disinfection with mouth rinses by targeting specifically oral malodor implicated anaerobic bacteria. Twenty healthy fasting subjects volunteered for a blinded prospective, descriptive correlational crossover cross-section clinical trial conducted during the month of Ramadan between July and August 2013 in Albaha province in Saudi Arabia involving the application of Listerine[R] Cool Mint[R] mouth rinse by either the traditional panoral rinsing method, or a site-specific disinfection method targeting the subgingival and supragingival plaque and the posterior third of the tongue dorsum, while avoiding the remaining locations within the oral cavity. The viable anaerobic and aerobic bacterial counts, volatile sulfur compounds [VSCs] levels, organoleptic assessment of oral odor, and the tongue-coating index were compared at baseline, one, 5, and 9 hours after the treatment. The site-specific disinfection method reduced the VSCs and anaerobic bacterial loads while keeping the aerobic bacterial numbers higher than the traditional panoral rinsing method. Site-specific disinfection can more effectively maintain a healthy oral cavity by predominantly disinfecting the niches of anaerobic bacteria within the oral cavity

Antecedentes: El efecto de los Compuestos Volátiles Sulfurados (CVS) sobre los tejidos periodontales, específicamente a nivel del eje RANKL/OPG, no ha sido dilucidado y en la actualidad existe escasa literatura al respecto publicada. Objetivo: Evaluar si los CVS medidos en la cavidad oral de pacientes con periodontitis crónica moderada a severa se relacionan con la expresión de RANKL y OPG a nivel de fluido gingival crevicular (FGC). Método: Se realizó un estudio transversal con 71 pacientes derivados de la Unidad de Diagnóstico de la Clínica Odontológica Docente Asistencial de la Universidad de los Andes. Posterior a la realización de un examen periodontal completo se tomaron muestra de los niveles de CVS de la boca de los pacientes mediante un monitor de sulfuros y muestras de FGC para evaluar los niveles de RANKL y OPG mediante test de ELISA. Los datos obtenidos fueron analizados mediante test de correlación de Spearman. Resultados: Al evaluar la correlación de los niveles de CVS con los niveles de RANKL, OPG y la razón RANKL/OPG, se observó un R de 0.098 con un p value = 0.41; -0.084 con un p value= 0.48 y 0.067 con un p value = 0.57 respectivamente. Conclusiones: El presente estudio no pudo demostrar si existe una relación entre los niveles de CVS con la expresión de RANKL y OPG en el FGC de pacientes con periodontitis crónica.


Background: The effect of the Volatile Sulfur Compounds (VSC) on the periodontal tissues, specifically at the RANKL/OPG level has not been elucidate and there is little literature published on this subject. Aim: To explore if the VSC levels measured in the oral cavity of patients with moderate or severe chronic periodontitis are correlated with the expression of RANKL and OPG at the gingival fluid level (GF). Method: A cross-sectional study was conducted with 71 patients referred by the Universidad de los Andes’s dentistry diagnosis department. After undergoing full-mouth periodontal exam, levels of CVS were recorded using a sulfur monitor and then GF was extracted to assess RANKL and OPG levels with ELISA. Data was analyzed with Spearman correlation test. Results: The correlation (R) between the VSC and the GF levels was 0.098 with a p value = 0.41; -0.084 with a p value = 0.48 y 0.067 with a p value = 0.57 respectively. Conclusions: This study could not demonstrate if there is a relation between the VSC levels and the expression of RANKL and OPG in the GF in patients with chronic periodontitis.

Introducción. El torus , o rodete, palatino es una alteración ósea benigna que puede generar molestias en la fonación y en la deglución. Cuando su crecimiento o exposición persistente producen síntomas molestos, debe ser resecado quirúrgicamente. Presentación del caso. Se trata de una paciente de 82 años de edad, que consultó por una lesión ulcerativa de un año de evolución, que producía dolor, halitosis y molestia a la deglución. En el examen físico de la cavidad oral, se observó una prominencia de consistencia dura en la línea media, con perforación y ulceración de la mucosa palatina en su porción lateral posterior izquierda. Se consideraron los diagnósticos de torus palatino, osteoma, granuloma piógeno y neoplasia de tejidos blandos. Se decidió resecarla quirúrgicamente y hacer el estudio histopatológico. La evolución clínica fue satisfactoria, con resolución completa un mes después de la cirugía. En el estudio de histopatología se observó hiperostosis ósea con inflamación crónica de los tejidos blandos e hiperplasia simple de la mucosa, que corresponden a un torus palatino. Discusión. Se reporta un inusual caso de exposición espontánea de un torus palatino que ocupaba casi toda la bóveda palatina, se describe su resección quirúrgica y se informan los hallazgos de histopatología. En la revisión bibliográfica realizada, no se encontró un reporte previo de exposición espontánea de torus palatino.


Introduction: Palatal torus, or torus palatinus, is a benign bone alteration that may cause some discomfort during phonation or swallowing. When its growth or persistent exposition produces unpleasant symptoms, it must be surgically removed. Case presentation: We treated an 82-year-old female patient who consulted for a painful ulcerous lesion she had had for a year and which produced halitosis and discomfort when swallowing. During the oral cavity physical exam we observed a hard protuberance on the midline. It showed a perforation and ulceration of the lateral posterior palatal mucosa on the left side. We considered the following diagnosis: palatal torus, osteoma, pyogenic granuloma, or a soft-tissue neoplasia. We decided to surgically remove it and to perform a histopathological examination. Clinical evolution was satisfactory with complete resolution one month after surgery. The histopathological examination showed hyperostosis along with chronic inflammation of the soft tissues and a simple hyperplasia of the mucosa, which in turn indicated a palatal torus. Discussion: We report an unusual case of spontaneous exposition of a palatal torus which took up almost all of the hard palate area. Its resection is described and we inform the histopathological findings. In the literature review, we did not find a previous report of a spontaneous exposition of a palatal torus.

Estudos têm demonstrado uma relação direta entre a doença periodontal e o odor ofensivo do hálito. O presente estudo teve como objetivo avaliar a frequência e distribuição de halitose em um grupo de pacientes com doença periodontal em um estudo transversal observacional (n=112) e, em um estudo intervencionista, avaliar o efeito do tratamento periodontal full-mouth e convencional na redução da halitose em um grupo de pacientes com doença periodontal. Os pacientes responderam a uma anamnese, tiveram seu hálito mensurado pelo halímetro e teste organoléptico, além de realizados Índice de placa visível, Índice de sangramento gengival, Índice de saburra lingual e exame periodontal completo. No estudo 2, os pacientes foram submetidos a seis distintas formas de tratamento: terapia periodontal em sessão única, terapia convencional em quadrantes e, um grupo controle, com somente instrução de higiene oral. Todas as modalidades subdivididas: com e sem raspagem lingual diária. No primeiro estudo os resultados mostraram que, tanto para teste organoléptico quanto para o halímetro, houve maior grau de halitose nos grupos de idades mais avançadas, nos que relataram sangramento gengival e escovação menos que três vezes ao dia. Ainda no teste organoléptico a escovação de língua gerou diferença estatística. Não houve diferença estatística entre as medidas de halitose entre teste organoléptico e halímetro. Foram encontrados aproximadamente 75% de pacientes periodontais com halitose. No segundo estudo os resultados mostraram superioridade conforme análise do halímetro para 30, 60 e 90 dias para os grupos de raspagem em sessão única contra raspagem por quadrantes. Patients answered an interview, they have had their breath collected by halimeter, they have evaluated by organoleptic test, and visible plaque index, gingival bleeding index, index of tongue coating and periodontal examination. In the second step, patients have been submitted to six different forms of treatment: periodontal therapy in one session, conventional therapy in quadrants, and the control group, with only oral hygiene instruction. All types split: with and without tongue scraping daily. The first study results shown for both organoleptic test as halimeter, a greater degree of halitosis in the older age groups, we have reported that gum bleeding and brushing less than three times per day. ABSTRACT

To compare the presence of Helicobacter pylori [H. pylori] infection by stool antigen test in children with and without halitosis. Comparative study. Department of Paediatrics, Fatih University Hospital, Ankara, Turkey, between December 2008 and June 2009. Fifty-three patients aged between 3-15 years who presented to paediatrics outpatient clinic with halitosis and 55 healthy children aged between 4-15 years without halitosis were included in the study. Halitosis was confirmed with organoleptic test. Stool antigen test was performed in both groups. Intergroup proportions were compared using chi-square and Fisher exact tests with significance at p < 0.05. The H.pylori stool antigen test was positive in 11 out of 53 patients [20.8%] with halitosis and 12 of 55 healthy controls [21.8%]. The rate of positive H. pylori stool antigen test results were similar between two groups [p > 0.05]. Two-weeks eradication treatment was administered to 11 patients with H. pylori infection and halitosis. After treatment, the symptoms of 8 patients with halitosis [72.7%] completely resolved and persisted in 3 patients [27.3%]. Seven of the 11 patients who were administered eradication treatment also had abdominal pain along with halitosis. Both symptoms completely resolved in all those patients after treatment. Although no statistically significant difference existed between the rate of H. pylori infections among those with and without halitosis. Eradication treatment was found beneficial in the treatment of children with halitosis and positive H. pylori stool antigen test

ABSTRACT

Halitosis is an unpleasant condition that causes social restraint. Studies worldwide indicate a high prevalence of moderate halitosis, whereas severe cases are restricted to around 5 percent of the populations. The etiological chain of halitosis relates to the presence of odoriferous substances in exhaled air, especially the volatile sulphur compounds (VSC) produced by bacteria. The organoleptic diagnosis is the gold standard and clinical management includes oral approaches, especially periodontal treatment and oral hygiene instructions, including the tongue. When oral strategies are not successful, referral to physicians is warranted.

OBJETIVO: Avaliar a relação etiológica entre a halitose e a presença de saburra lingual. MÉTODO: Os métodos utilizados foram entrevistas, com preenchimento de uma ficha clínica apropriada; exame clínico (inspeção visual e sondagem periodontal) e aferição do grau de halitose, utilizando o aparelho Breath Alert. O universo desta pesquisa compreendeu os indivíduos atendidos nas clínicas integradas do Departamento de Odontologia da Universidade Estadual da Paraíba, no período destinado à coleta de dados, sendo totalizada uma amostra de 80 pacientes. RESULTADOS: Foi constatado que 66,0 por cento dos pacientes investigados relataram realizar higiene da língua, 32,5 por cento relataram não higienizá-la e 1,5 por cento afirmaram higienizá-la apenas esporadicamente. Verificou-se também que 41,7 por cento dos investigados apresentaram odor leve no hálito e 60,0 por cento daqueles que apresentaram odor moderado exibiram a presença de saburra lingual. No entanto, 52,4 por cento dos pacientes classificados sem odor no hálito também apresentaram tal alteração na língua. A prevalência da halitose na população do estudo foi de apenas 21,3 por cento, na qual 15,0 por cento apresentaram odor leve e 6,3 por cento odor moderado no hálito. CONCLUSÃO: Apesar de a literatura relatar que os fatores que facilitam o acúmulo de biofilme dental na língua, resultando em saburra lingual, influenciam no desenvolvimento e na instalação da halitose, não foi verificado relação significativa, neste experimento, entre a presença de saburra lingual e a ocorrência da halitose.


OBJECTIVE: This study assessed the relationship between halitosis and white tongue. METHODS: The patients were interviewed and a clinical form was filled out. They were also examined by visual inspection and periodontal probing. The degree of halitosis was determined by the Breath Alert device. The universe of this study consisted of individuals who visited the clinics of the State University of Paraiba Dental Department during the study period. The sample consisted of 80 patients. RESULTS: Sixty-six percent of the patients reported cleaning their tongues, 32.5 percent reported never cleaning their tongues and 1.5 percent reported cleaning their tongues periodically. Forty-one point seven percent of the patients with mild halitosis and 60,0 percent of those with moderate halitosis presented white tongue. However, 52.4 percent of the patients without halitosis also presented white tongue. Only 21.3 percent of the sample presented halitosis; 15,0 percent had mild halitosis and 6.3 percent had moderate halitosis. CONCLUSION: According to the literature, the factors that promote the accumulation of dental biofilm on the tongue, resulting in white tongue, also promote the development and establishment of halitosis. However, the present study did not find a significant relationship between white tongue and halitosis.
